HERE’S WHAT YOU NEED TO DO

Step 1: Download the IRA kit below to open a Traditional, Roth or SEP IRA.

Step 2: Complete, print and sign the forms.

Step 3: Fax or mail your completed, signed forms to SDIRA:

Be Sure to Include:

  • A copy of your valid driver’s license, state-issued photo ID or current passport.
  • The form(s) you completed and signed from the IRA kit.
  • Your check for the 1st year fee of $60, $80 or $100.

Important:
If you fax your new account opening forms to SDIRA Services and you are requesting a direct transfer from another IRA custodian, you must mail your completed & signed Transfer Request form to SDIRA Services. The releasing custodian will require your original signature and our acceptance signature before it will process your transfer.
